APPLY FOR CREDIT

The above signed hereby authorizes All-In Truck Parts of Indiana to fully investigate all credit information available on applicant, and set reasonable credit limits on their account, if established. The above sign agrees to the following terms: Statements are rendered the 1st of the month and they are due by the 10th of the month. If we have not received payment by the 20th of the following month, the account will be classified as delinquent, and it will be placed on a credit hold status. Accounts past 30 days will be assessed a finance charge of 1.5% per month (18% ARP). All collection and/or attorney fees will be added to account balance. The undersigned understands that All-In Truck Parts of Indiana and/or its assignees will not process a credit order until after All-In Truck Parts of Indiana or such assignees have conducted a credit investigation and determined, in its or their sole discretion, to approve the extension of credit to the undersigned. By submitting this Credit Application, the undersigned hereby authorizes a full and complete investigation by All-In Truck Parts of Indiana and its assignees into the undersigned’s credit history. The undersigned further authorizes All-In Truck Parts of Indiana and its assignees to conduct an investigation into the credit history of the undersigned’s proprietors, partners, shareholders, members and/or principals as All-In Truck Parts of Indiana and/or its assignees see fit in its or their sole discretion. The undersigned hereby grants permission to all of the above credit references and further authorizes the use of this document (or a facsimile thereof) as consent for the release to All-In Truck Parts of Indiana and its assignees of credit information by credit reporting agencies, credit bureaus, and the credit, banking and trade references supplied to All-In Truck Parts of Indiana and its assignees by the undersigned. All-In Truck Parts of Indiana and its assignee shall be further authorized to conduct periodic credit checks and reviews of the credit history of the undersigned and its proprietors, partners, shareholders, members and/or principals in order to continue the extension of credit to the undersigned.
